1	Withdraw Method	Select withdraw method, default is cash.
2	Payment Date	You can set previous date but it is not mandatory to set date. If you don't, it will auto-set to today's date.
3	Cheque Info	If you select cheque in payment method, you can set cheque info here.
4	Note	Insert payment note.
5	Amount	Insert negative amount.
